Cancellation Policy

Participants must acknowledge the following policies: ☐ I understand that all class sales are final and non-refundable. ☐ I understand that a minimum of five (5) participants is required for class to run. If the minimum is not met, the class may be rescheduled or a class credit will be issued. ☐ I understand that if I cannot attend, I must notify Lia Choreography at least two (2) hours before the class start time to receive a class credit. ☐ I understand that credits are valid for 30 days only and cannot be transferred or refunded. ☐ I understand that no-shows or late cancellations (less than 2 hours before class) will result in forfeiture of the class fee. I understand that participation in dance classes, rehearsals, and workshops with Lia Choreography involves physical activity that carries the risk of injury. By signing this form, I acknowledge and agree that: • I voluntarily participate in these activities. • I am physically able to participate in dance instruction and movement activities. • I assume full responsibility for any risk of injury, illness, or damages that may occur during participation. I hereby release, waive, and discharge Lia Choreography, its instructors, staff, and affiliates from any and all liability, claims, or demands arising from injury, illness, or damages sustained during participation in classes or related activities. I understand that Lia Choreography is not liable for injuries, lost items, or damages that may occur during or after participation in class. I give permission for Lia Choreography to photograph or record video during classes and use this content for promotional purposes, social media, and marketing. By signing below, I confirm that I have read and agree to all class policies, attendance rules, and the liability waiver listed above.
